Exceptional Family Member Program

An Exceptional Family Member (EFM) is one with any physical, emotional, developmental, or intellectual disability which requires special treatment, therapy, education, training, or counseling.

The EFM Program works with other military and civilian agencies to provide comprehensive and coordinated medical, education, housing, community support, and personnel services to families with special needs.

Enrollment in the EFMP is mandatory whether the Exceptional Family Member is an adult or a child.

Enrollment in EFMP does not adversely affect selection for promotion, schools, or assignment. Information concerning enrollment is not made available to selection boards.

Enrollment allows assignment managers at Army personnel agencies to consider the documented medical and special education needs of a soldier's Exceptional Family Member during the assignment process. When possible, soldiers are assigned to an area where the medical and special education needs of their Exceptional Family Member can be met. This is dependent on a valid personnel requirement for the soldier’s grade, specialty, and eligibility for the tour.

All soldiers are still eligible for world wide assignments.

Special education needs are considered outside the United States only. Assignments within the fifty states and Trust Territories are not based on the educational needs of children. Every local school system must obey public laws regarding the provision of special education.

To enroll in Exceptional Family Member Program:  Contact the EFMP point of contact at the nearest medical treatment facility (MTF), to get the forms and begin the assessment process. Once completed, MTF staff will forward the forms to the  regional medical center for coding and on to Army personnel agencies who enroll the soldier.

EFM Medical Summary

 

If you do not have medical records for when you get your screening done you will need:

Women -- Current Pap Smear results and a physical dated within one year.

Kids -- Most current well check up report and current immunization records.

 

EFMP Process Checklist

  • Obtain DA 5888, 7246, 2792 or 2792-1 from Unit PAC or the ACS EFMP Manager.

  • Completes Part A of the DA Form 5888. This form must be authenticated in Part B by PSB before the screening process can begin.

  • Call the Medical Case Coordinator to schedule the EFMP screening appointment and let them know the purpose of the screening (COT, re-enlistment, medical condition etc.) Make sure all paperwork is completed BEFORE making your appointment.

  •  The Medical Case Coordinator will collect 7246 and if enrollment is warranted the 2792. If educational enrollment is warranted DODDs will complete the 2792-1. A doctor must sign in block 10 of the 5888. You need to take the original 5888-R back to your S-1 and then to your PSB office.

  • It is important that you obtain copies of all EFMP paperwork for your personal records.

  • If family members go to ACS to in process with the EFMP Manager and to obtain valuable information on your rights and responsibilities and support services available to you in the community.
